Recorded scores drive the improvement arc; where a score was never recorded the line stays blank with an amber chip (dictate one line + Re-run to fill it — numbers are never made up, keeping the notes audit-safe). Patients 2+ months inactive get a 📄 Discharge summary button. Review, then Approve & upload finalises in Cliniko under that physio's login.
